Vaccine Side Effects Sick Day Form

Sick Day Buy Back Form

Anyone who had to use a sick day due to side effects from the COVID-19 vaccination is able to receive that sick day back.  You will need to print out and fill out the attached form.  You need to:

• Print out, complete and sign the form
  
• Supervisors/Principals will sign off on the form
  
• The form would go to Marsha in payroll (similar to a timesheet)

Please check your pay stub after a couple of pay cycles after passing in the form to double check that the sick day has been given back to you.
